H1N1 Clinic Set for Wednesday at Cudahy High School
The Cudahy Health Department has scheduled a second H1N1 flu clinic, this time for residents who fall in an expanded group of high-risk categories.
The clinic will be held Wednesday, November 18th from 3:30 p.m. until 7:30 p.m. at Cudahy High School (4950 South Lake Drive). People are asked to enter the parking lot from East Pulaski Avenue then meet at the entrance to the school. You will be directed where to go from there.
This H1N1 clinic is for health care providers, pregnant women, parents of children under the age of six months, children between six months and 18 years of age and adults between 19 and 64 who have underlying conditions that could cause complications from the H1N1 flu.
Please note that biological parents must be present with their children. In addition, a consent form must be completed before the H1N1 flu shot can be administered. Forms can be downloaded from the City of Cudahy website, www.cudahy-wi.gov and the Cudahy School district website, www.cudahy.k12.wi.us.
“We ask that people try to fill out their consent forms in advance,” said Health Officer Carol Wantuch. “This will help traffic move more quickly and reduce the wait time.”
